Description
The program is a structured, therapeutic program that assists in transitioning patients from an inpatient hospital setting or a as a standalone service to a more independent life in the community as well patient’s seeking an outpatient environment. The program is designed to increase and maintain the therapeutic gains individuals have made as patients in the hospital and to address mental health issues. It may also help prevent the need for future hospitalizations. Tailored to meet each individual’s needs and provides direction and tools that each participant can use in their daily lives. Curriculum provides life sills, abstinence-based coping skills, referral services, and peer support. This is a safe, secure environment conducive to recovery.
