A team of professionals and natural supports work together to create a plan. - Reduces the impact of mental health symptoms - Increases resiliency and supports long-term recovery - Keeps youth safe at home and successful in school - Prevents involvement with the juvenile justice system - Supports positive youth development and independence The team typically includes natural supports like family, friends, and community leaders as well as professionals such as therapists, school staff, CPS, and probation officers.
