Harm Reduction Program offered by Cowlitz Family Health Center at Grade Street Campus

Provides harm reduction services and wrap-around support for people accessing syringe exchange. Offers one-to-one exchange of clean syringe equipment, along with health education, wound care, and naloxone for overdose prevention. Includes Hepatitis C and HIV screening with referral for care, family planning services such as birth control counseling, contraception (Depo-Provera), emergency contraception, and pregnancy testing. Also helps with Medicaid enrollment and provides referrals for primary care, dental care, substance use disorder treatment, medication-assisted treatment, and other community services.

- None for syringe exchange, health education, or Naloxone. - Accepts Medicare, Medicaid (Washington Apple Health), Community Health Plan of Washington, Molina, and Coordinated Care. - Accepts private insurance including United Healthcare and Amerigroup. - Offers sliding scale fee for wound care, HIV and Hepatitis C testing, and family planning.
