Focuses on serving individuals and families who may be experiencing frequent hospitalizations, interactions with law enforcement, or involvement with child protective services.

Provides integrated primary care and behavioral health care services. Focuses on both preventative care and chronic disease management. Offers psychiatric assessments which can identify certain diagnoses and treatment, including medications, which will be managed by the primary care provider. Offers a variety of therapy options for existing patients as well as treatment for substance use disorder. Able to address moderate to severe mental and behavioral health needs.
