Children in Grant County who:
  • Are 20 years old or under
  • Have complex mental or behavioral health needs
  • Are Medicaid-eligible
  • Complete a CANS screening (Child and Adolescent Needs and Strengths tool)
  • Meet Access to Care Standards, which determine the right level of care

Offers a voluntary, home- and community-based program that supports youth and their families who are experiencing complex behavioral health challenges. Services are delivered by a collaborative team and are designed to help families thrive in the least restrictive setting possible—often right in their home or community. Brings together natural supports (like family members, friends, or faith leaders) and professional team members (therapists, care coordinators, school staff, probation officers, etc.) to build a plan tailored to the child and family’s needs.

Provides counseling services that include mental health, substance use recovery, crisis stabilization, and developmental disability. Other services include case management, employment referrals, housing assistance, peer support, Wraparound Intense Services (WISe), and psychotropic medication management. Serves those on Medicaid and have reduced fees for low income individuals.
