Application process
To schedule an appointment for a drug/alcohol evaluation: You need to come to our office, sign release papers and pre-pay for an appointment at a future date. This takes about 10-15 minutes. Those unable to come into the office can set up an appointment through email/fax and pay with a credit/debit card.
