Call to schedule an appointment. Bring the proper documentation. To save time, new patients may fill out their Patient Registration Form online.

Fees vary by service. Most insurances are accepted including Medicare, Apple Health (Medicaid), Community Health Plans of Washington, and private insurance. A sliding fee scale is available.
