Provides integrated behavioral health care to adults, children and families with intellectual and developmental disabilities. Creates customized treatment plans that may include mental health and substance abuse assessment and treatment, medications, and supports such as: - Individual, couple, family, or group counseling sessions - Trauma-related services - Medication monitoring and management - Nursing services - Peer support - Crisis support - Employment support

Eligibility
Those ages 6 and older in King, Pierce and Snohomish counties with behavioral health needs combined with intellectual and developmental disabilities - particularly those that significantly impact physical abilities, learning, language, and behavior. Examples of disabilities include: - Intellectual Disability - Autism Spectrum Disorder - Cerebral Palsy - Down Syndrome - Fragile X Syndrome
