Also helps with Medicaid enrollment and provides referrals for primary care, dental care, substance use disorder treatment, medication-assisted treatment, and other community services.

Laboratory services are available.  Also offers diabetes education. Refers to specialty, laboratory and diagnostic testing services and treatment when appropriate.
